Preventive check-ups and screening tests: why they matter and which ones you need

Why prevention matters so much

Many of the most common killers — heart disease, stroke, diabetes and several cancers — develop silently for years. High blood pressure, high cholesterol and early colon polyps usually cause no symptoms. By the time something hurts, the disease is often advanced. Screening flips this: it looks for problems in people who feel well, so they can be treated early or even prevented. A polyp removed during colonoscopy never becomes cancer; high blood pressure treated at age 50 never causes the stroke it might have caused at 65.

The evidence, test by test

Summary of the 2022 Council of the European Union Recommendation on cancer screening and major guidelines [9]. In Greece, the national programme «Προλαμβάνω» offers free screening tests (e.g. mammography, Pap/HPV, stool test) for eligible age groups — check gov.gr or the myHealth app.

Screening has limits too

  • False alarms: a positive result often leads to further tests that turn out normal — stressful, but part of the process.
  • Over-diagnosis: some slow-growing cancers (for example some prostate or breast cancers) would never have caused harm; treating them brings side effects without benefit.
  • More is not always better: a Cochrane review of general health checks — broad check-ups in healthy adults — found no reduction in overall or cardiovascular deaths [8]. The value lies in the specific tests proven by trials, done at the right age and interval.

Don't wait for screening if you have symptoms. Blood in the stool, a new breast lump, unexplained weight loss, a cough lasting over three weeks, or chest pain need a doctor now — not at your next scheduled check-up.

Frequently asked questions

I feel fine — why should I get tested? Because the diseases screening looks for usually give no symptoms until they're advanced. Feeling well is exactly when screening works best.

Does family history change things? Yes. A close relative with early heart disease, colon or breast cancer can mean starting earlier or testing more often.

Is a yearly "full blood panel" necessary? Not for most healthy adults. Ask for the tests that match your age and risk.

Γιατί η πρόληψη μετράει τόσο πολύ

Πολλοί από τους συχνότερους «δολοφόνους» — καρδιοπάθεια, εγκεφαλικό, διαβήτης και αρκετοί καρκίνοι — αναπτύσσονται σιωπηλά για χρόνια. Η υψηλή πίεση, η ψηλή χοληστερίνη και οι πρώιμοι πολύποδες του εντέρου συνήθως δεν δίνουν συμπτώματα. Όταν κάτι πονέσει, η νόσος συχνά έχει προχωρήσει. Ο προληπτικός έλεγχος το αντιστρέφει: ψάχνει προβλήματα σε ανθρώπους που νιώθουν καλά, ώστε να θεραπευτούν νωρίς ή και να προληφθούν. Ένας πολύποδας που αφαιρείται στην κολονοσκόπηση δεν γίνεται ποτέ καρκίνος· μια ψηλή πίεση που ρυθμίζεται στα 50 δεν προκαλεί ποτέ το εγκεφαλικό που θα μπορούσε να προκαλέσει στα 65.

Σύνοψη της Σύστασης του Συμβουλίου της ΕΕ (2022) για τον έλεγχο καρκίνου και βασικών κατευθυντήριων οδηγιών [9]. Στην Ελλάδα, το εθνικό πρόγραμμα «Προλαμβάνω» προσφέρει δωρεάν προληπτικές εξετάσεις (π.χ. μαστογραφία, Παπανικολάου/HPV, τεστ κοπράνων) στις ηλικίες που δικαιούνται — δες στο gov.gr ή στην εφαρμογή myHealth.

Και ο έλεγχος έχει όρια

  • Ψευδείς συναγερμοί: ένα θετικό αποτέλεσμα συχνά οδηγεί σε επιπλέον εξετάσεις που βγαίνουν φυσιολογικές — αγχωτικό, αλλά μέρος της διαδικασίας.
  • Υπερδιάγνωση: ορισμένοι αργοί καρκίνοι (π.χ. κάποιοι του προστάτη ή του μαστού) δεν θα έβλαπταν ποτέ· η θεραπεία τους φέρνει παρενέργειες χωρίς όφελος.
  • Περισσότερο δεν σημαίνει πάντα καλύτερο: ανασκόπηση Cochrane για τα γενικά τσεκ-απ υγιών ενηλίκων δεν βρήκε μείωση στους συνολικούς ή καρδιαγγειακούς θανάτους [8]. Η αξία βρίσκεται στις συγκεκριμένες εξετάσεις που έχουν αποδειχθεί σε μελέτες, στη σωστή ηλικία και συχνότητα.

Μην περιμένεις τον προγραμματισμένο έλεγχο αν έχεις συμπτώματα. Αίμα στα κόπρανα, νέο ογκίδιο στον μαστό, ανεξήγητη απώλεια βάρους, βήχας πάνω από τρεις εβδομάδες ή πόνος στο στήθος θέλουν γιατρό τώρα.

Συχνές ερωτήσεις

Νιώθω μια χαρά — γιατί να εξεταστώ; Γιατί οι παθήσεις που ψάχνει ο έλεγχος συνήθως δεν δίνουν συμπτώματα μέχρι να προχωρήσουν. Όταν νιώθεις καλά είναι ακριβώς η στιγμή που ο έλεγχος πιάνει καλύτερα.

Αλλάζει κάτι το οικογενειακό ιστορικό; Ναι. Στενός συγγενής με πρώιμη καρδιοπάθεια ή καρκίνο εντέρου ή μαστού μπορεί να σημαίνει νωρίτερο ή συχνότερο έλεγχο.

Χρειάζομαι κάθε χρόνο «όλες τις εξετάσεις αίματος»; Όχι οι περισσότεροι υγιείς ενήλικες. Ζήτα τις εξετάσεις που ταιριάζουν στην ηλικία και στον κίνδυνό σου.
