Anxiety and stress: symptoms, when it becomes a disorder and what helps

Stress, worry and anxiety — what's the difference?

Stress is the body's reaction to a demand — a deadline, a bill, an argument. It usually eases when the situation passes. Anxiety is a feeling of fear or unease about something that might happen; it is normal and even useful in small doses, because it makes us prepare. It becomes an anxiety disorder when it is excessive, lasts for months, is hard to control and interferes with work, relationships or sleep.

Common types include generalised anxiety disorder (persistent worry about many things), panic disorder (sudden attacks of intense fear with physical symptoms), social anxiety and specific phobias.

Physical symptoms people often don't recognise

  • Fast or pounding heartbeat, chest tightness, shortness of breath.
  • Stomach pain, nausea, diarrhoea, "knot" in the stomach.
  • Muscle tension, headaches, jaw clenching, tiredness.
  • Dizziness, tingling in hands or lips (often from over-breathing).
  • Trouble falling asleep, waking with racing thoughts.

Chest pain and breathlessness should always be checked by a doctor first — especially if you are older, smoke or have other risk factors. A panic attack is diagnosed after heart and other causes have been ruled out.

A quick self-check: the GAD-7

Doctors often use a 7-question screening tool, the GAD-7 [2]. It asks how often, over the last two weeks, you have been bothered by: feeling nervous or on edge; not being able to stop worrying; worrying too much about different things; trouble relaxing; being so restless it's hard to sit still; becoming easily annoyed; and feeling afraid something awful might happen. Each item is scored 0–3. Totals of 5, 10 and 15 roughly correspond to mild, moderate and severe anxiety. A score of 10 or more is a good reason to talk to a professional. It is a screening tool, not a diagnosis.

What helps — what the research shows

Pooling 1,039 trials, physical activity reduced symptoms of depression, anxiety and distress. Benefits were seen across many types of activity — walking, resistance training, yoga — and higher-intensity activity tended to have greater effects.

Across 41 placebo-controlled trials, CBT for anxiety-related disorders had moderate effects compared with placebo conditions, with the strongest effects for obsessive-compulsive disorder, generalised anxiety and acute stress.

An 8-week mindfulness-based stress reduction course was non-inferior to the antidepressant escitalopram for reducing anxiety in adults with anxiety disorders — a well-tolerated option for some people.

Everyday steps that make a difference

  1. Move most days — even 20–30 minutes of brisk walking.
  2. Slow breathing: breathe in through the nose for about 4 seconds and out slowly for about 6. Longer exhalations calm the body's alarm system.
  3. Limit caffeine and alcohol. Caffeine can trigger palpitations and panic-like symptoms; alcohol relieves anxiety briefly but worsens it the next day ("rebound").
  4. Protect your sleep — anxiety and poor sleep feed each other.
  5. Schedule "worry time": write worries down and deal with them at a set time, rather than all day.
  6. Stay connected — talk to people you trust; avoid withdrawing.

When to see a professional

Seek help if anxiety lasts for weeks, stops you doing things you need or want to do, causes panic attacks, or if you use alcohol or pills to cope. A GP, psychiatrist or psychologist can confirm the diagnosis and offer CBT, other therapies or medication (such as SSRIs). Medication and therapy can be combined, and both work — the right choice depends on the person.

Frequently asked questions

Can anxiety cause real physical symptoms? Yes. Adrenaline and the stress response genuinely change heart rate, breathing, digestion and muscles.

Are "calming" supplements effective? Evidence for most supplements is weak or inconsistent, and some interact with medicines. Discuss them with your doctor or pharmacist.

Is anxiety a weakness? No. It is a common, treatable health condition — like high blood pressure.

Στρες, ανησυχία και άγχος — ποια η διαφορά;

Το στρες είναι η αντίδραση του οργανισμού σε μια απαίτηση — μια προθεσμία, ένας λογαριασμός, ένας καβγάς. Συνήθως υποχωρεί όταν περάσει η κατάσταση. Το άγχος είναι αίσθημα φόβου ή ανησυχίας για κάτι που μπορεί να συμβεί· είναι φυσιολογικό και σε μικρές δόσεις χρήσιμο, γιατί μας κάνει να προετοιμαστούμε. Γίνεται αγχώδης διαταραχή όταν είναι υπερβολικό, κρατά μήνες, δύσκολα ελέγχεται και επηρεάζει δουλειά, σχέσεις ή ύπνο.

Συχνές μορφές είναι η γενικευμένη αγχώδης διαταραχή (επίμονη ανησυχία για πολλά θέματα), η διαταραχή πανικού (ξαφνικές κρίσεις έντονου φόβου με σωματικά συμπτώματα), το κοινωνικό άγχος και οι ειδικές φοβίες.

Σωματικά συμπτώματα που συχνά δεν αναγνωρίζουμε

  • Γρήγορος ή δυνατός χτύπος καρδιάς, σφίξιμο στο στήθος, δύσπνοια.
  • Πόνος στο στομάχι, ναυτία, διάρροια, «κόμπος» στο στομάχι.
  • Μυϊκή ένταση, πονοκέφαλοι, σφίξιμο σαγονιού, κόπωση.
  • Ζάλη, μυρμήγκιασμα σε χέρια ή χείλη (συχνά από υπεραερισμό).
  • Δυσκολία να κοιμηθείς, ξύπνημα με σκέψεις που «τρέχουν».

Ο πόνος στο στήθος και η δύσπνοια πρέπει πάντα να ελέγχονται πρώτα από γιατρό — ειδικά αν είσαι μεγαλύτερος σε ηλικία, καπνίζεις ή έχεις άλλους παράγοντες κινδύνου. Η κρίση πανικού διαγιγνώσκεται αφού αποκλειστούν η καρδιά και άλλες αιτίες.

Γρήγορος αυτοέλεγχος: το GAD-7

Οι γιατροί χρησιμοποιούν συχνά ένα ερωτηματολόγιο 7 ερωτήσεων, το GAD-7 [2]. Ρωτά πόσο συχνά, τις τελευταίες δύο εβδομάδες, σε ενόχλησαν: νευρικότητα ή ένταση· αδυναμία να σταματήσεις να ανησυχείς· υπερβολική ανησυχία για διάφορα· δυσκολία να χαλαρώσεις· τόση ανησυχία που δύσκολα κάθεσαι ήσυχος· εύκολος εκνευρισμός· φόβος ότι θα συμβεί κάτι κακό. Κάθε ερώτηση βαθμολογείται 0–3. Σύνολα 5, 10 και 15 αντιστοιχούν περίπου σε ήπιο, μέτριο και σοβαρό άγχος. Βαθμολογία 10 ή πάνω είναι καλός λόγος να μιλήσεις με ειδικό. Είναι εργαλείο ελέγχου, όχι διάγνωση.

Τι βοηθά — τι δείχνει η έρευνα

Συνδυάζοντας 1.039 μελέτες, η σωματική δραστηριότητα μείωσε τα συμπτώματα κατάθλιψης, άγχους και ψυχικής πίεσης. Όφελος φάνηκε σε πολλά είδη κίνησης — περπάτημα, ενδυνάμωση, γιόγκα — και η πιο έντονη δραστηριότητα έτεινε να έχει μεγαλύτερο αποτέλεσμα.

Ένα πρόγραμμα 8 εβδομάδων μείωσης στρες με ενσυνειδητότητα (mindfulness) ήταν εξίσου αποτελεσματικό με το αντικαταθλιπτικό εσιταλοπράμη στη μείωση του άγχους σε ενήλικες με αγχώδεις διαταραχές — μια καλά ανεκτή επιλογή για κάποιους.

Καθημερινά βήματα που κάνουν διαφορά

  1. Κινήσου τις περισσότερες μέρες — έστω 20–30 λεπτά γρήγορο περπάτημα.
  2. Αργή αναπνοή: εισπνοή από τη μύτη για περίπου 4 δευτερόλεπτα και αργή εκπνοή για περίπου 6. Η μεγαλύτερη εκπνοή ηρεμεί το «σύστημα συναγερμού» του σώματος.
  3. Λιγότερη καφεΐνη και αλκοόλ. Ο καφές μπορεί να πυροδοτήσει ταχυπαλμίες και συμπτώματα σαν πανικό· το αλκοόλ ανακουφίζει προσωρινά αλλά χειροτερεύει το άγχος την επόμενη μέρα.
  4. Προστάτεψε τον ύπνο σου — άγχος και κακός ύπνος τροφοδοτούν το ένα το άλλο.
  5. Όρισε «ώρα ανησυχίας»: γράψε τις σκέψεις που σε απασχολούν και ασχολήσου μαζί τους σε συγκεκριμένη ώρα, όχι όλη μέρα.
  6. Μείνε σε επαφή — μίλα με ανθρώπους που εμπιστεύεσαι· μην απομονώνεσαι.

Πότε να δεις ειδικό

Ζήτησε βοήθεια αν το άγχος κρατά εβδομάδες, σε σταματά από πράγματα που πρέπει ή θέλεις να κάνεις, σου φέρνει κρίσεις πανικού, ή αν καταφεύγεις σε αλκοόλ ή χάπια για να το αντέξεις. Ο γενικός γιατρός, ο ψυχίατρος ή ο ψυχολόγος μπορούν να επιβεβαιώσουν τη διάγνωση και να προτείνουν CBT, άλλες θεραπείες ή φάρμακα (π.χ. SSRIs). Φάρμακα και ψυχοθεραπεία συνδυάζονται και δουλεύουν και τα δύο — η σωστή επιλογή εξαρτάται από τον άνθρωπο.

Συχνές ερωτήσεις

Μπορεί το άγχος να δώσει πραγματικά σωματικά συμπτώματα; Ναι. Η αδρεναλίνη και η αντίδραση στρες αλλάζουν πραγματικά σφυγμούς, αναπνοή, πέψη και μύες.

Τα «ηρεμιστικά» συμπληρώματα δουλεύουν; Οι αποδείξεις για τα περισσότερα είναι αδύναμες ή αντιφατικές, και κάποια αλληλεπιδρούν με φάρμακα. Συζήτησέ τα με γιατρό ή φαρμακοποιό.

Το άγχος είναι αδυναμία; Όχι. Είναι συχνή κατάσταση υγείας που θεραπεύεται — όπως η υπέρταση.
